Objective: The polycystic ovary syndrome (PCOS) shares some components of metabolic syndrome. Thus, women with PCOS are at high risk for the possible development of early-onset cardiovascular disease (CVD). Increased levels of plasma homocysteine (pHcy) were shown to be a significant risk factor for CVD and are associated with insulin resistance (IR). Therefore, we investigated the correlation of pHcy with IR and obesity in patients with PCOS. Material and Methods: A total of 36 women with PCOS were divided into two groups according to body mass index (BMI) as group 1 (nonobese: BMI <25 kg/m2) and group 2 (overweight and obese: BMI >25 kg/m2). Two groups were compared with each other and the control group for basal hormones, pHcy, lipids, fasting blood insulin (FI) and glucose (FG), vitamine B12, folic asit, BMI, waist to hip ratio (WHR) and homeostasis model assessment (HOMA-R). Correlations of pHcy with IR and BMI were analyzed. Results: Basal hormone levels (LH/FSH, estradiol, testosterone, DHEA-S, cortisol), pHcy, FI, HOMA-R, triglyceride, Ferriman Gallwey Score (FGS) and diastolic blood pressure in PCOS were significantly higher than those in controls. FG level was not different. HDL was low. Obese patients with PCOS had higher HOMA-R, FI, testosterone, estradiol, triglyceride and FGS than nonobese patients. PHcy was significantly correlated with IR and FI but not with BMI, WHR, age, FG and FGS. Conclusion: IR, atherogenic dyslipidemia, obesity and elevated pHcy in PCOS indicate a high risk for CVD. Correlations of pHcy with IR may be noticeable for a prediction of premature atherosclerosis.
